Integrated Care Management is seeking a compassionate and experienced Patient Care Manager to lead and coordinate high-quality home-based care services for our clients. Key Responsibilities
• Coordinate and oversee individualized patient care services
• Develop, implement, and monitor comprehensive care plans
• Initiate and manage treatment interventions
• Recruit, supervise, and support caregiving staff
• Monitor service delivery to ensure quality and compliance
• Conduct client intake and comprehensive assessments
• Evaluate eligibility and admission needs
• Provide ongoing follow-up and problem resolution for clients and families
Qualifications
• Bachelor’s degree (4-year) in Nursing, Healthcare Management, Social Work, or related field
• Experience in care coordination, case management, or home health preferred
• Strong supervisory and leadership skills
• Excellent communication and interpersonal abilities
• Proficiency in documentation and care planning
• Ability to manage multiple priorities and problem-solve effectively
